Tell us about your home
If the form is slow or difficult to use, open it in a separate tab or call 888-400-1045.
The form is provided by Jotform. Submit only the information requested. A request does not bind coverage or confirm an effective date.
What we will ask
- The address, the year built and the square footage.
- How the house is built (frame, brick, stone or stucco) and the foundation type.
- The roof material and the year it was installed.
- The electrical panel, service size and any knob-and-tube, fuses or aluminum wiring, with update dates.
- Plumbing materials, the water heater’s age and any past leaks.
- The heating system, fuel and any oil tank or wood stove.
- How the house is used: primary home, seasonal, rented, vacant or under renovation.
- Your current insurer, coverage amounts and renewal date, plus any claims in the last five years.
- Any nonrenewal or cancellation notice, and its effective date.
- Historic status, local district rules or a preservation easement.
- Valuables such as antiques, art or collections.
Unknown answers are fine. Guesses are not, because an inaccurate application can cause problems at claim time.
Have these ready if you can
- Your current declarations page.
- Invoices or permits for the roof, electrical, plumbing and heating.
- Photos of each side of the house, the roof, the electrical panel and the water heater.
- A four-point inspection or other recent inspection report.
- A rebuilding estimate or appraisal.
Not sure where to start? The quote preparation checklist builds a list for your house from the details you choose, and the quote checklist covers everything in detail.
How it works
- We talk through the house, your timeline and what matters most to you.
- We look at which kinds of insurers may fit: standard, specialty or surplus lines.
- If options are available, compare the actual proposals and policy terms alongside the price.
- Confirm binding, the effective date and any conditions through the authorized insurance process before relying on coverage.
Coverage, availability and price depend on the insurer, the property and your state. We do not take quote inquiries for homes in California, Hawaii, New York and Rhode Island.
